PILES OR HAEMORRHOIDS:- Piles are inflammation of tissue and veins in the lower part of rectum. They can vary in size and can be found inside and outside the anal canal. It is estimated that 50% of people tend to face this condition at some point in their lives. Males and females are equally affected but people feel shy while discussing this medical problem and delay in seeking medical treatment worsens their condition. CLASSIFICATION OF PILES:- The internal piles are more common and they are located 2-4 cm above anal verge. They are caused by dilation of internal hemorrhoidal plexus. Mainly they are found at 3, 7 and 11 o'clock positions and at these positions they are called primary piles. In some cases their position may vary and if position varies then they are called secondary piles. Internal piles are graded from grade i to iv. Grade-I :- No mass protrusion from anal canal, on and off per rectal bleeding during and immediately after defecation is there. Grade-II:- There is protrusion of mass from anal canal during defecation and it simultaneously reduce by itself after defecation. Bleeding sign is the same as grade I. Grade-III:- In grade III protrusion of mass from anal canal during defecation is there but it doesn't reduce by itself. It has to be pushed back manually. Bleeding sign is the same as grade I. Grade-IV:- In grade iv protrusion of mass from anal canal is always there, it can't be reduced. If it is pushed back it again comes out. Bleeding sign is the same as grade I. External piles occur outside the anal verge. They are caused by dilation of the external hemorrhoidal plexus. CAUSES OF PILES:- Piles are caused due to increased pressure in the lower part of rectum that result in stretching of blood vessels around anus and due to that inflammation of these blood vessels and surrounding tissues form piles. Main causes are Chronic constipation Chronic diarrhea Straining due to lifting of heavy weights Pregnancy- increased size of uterus applies pressure on lower part of abdominal cavity. Obesity. Fast foods with low fiber contents Sedentary lifestyle Straining while passing stool Secondary to portal hypertension SYMPTOMS OF PILES:- External piles are painful and patient need immediate medical attention. Per rectal bleed and swelling (hematoma) at anal verge is there. Internal piles are painless. There is dark red bleeding with stool and after stool on and off. Sometimes mass protrusion outside the anus and if it remains outside then it is called prolapsed piles. If prolapsed piles get strangled then it is painful. An itch and burning sensation in some patients is there. TREATMENT:- • External piles can be managed conservatively with medications, cold compression but if conservative management fail, need immediate incision and drainage of hematoma. • Internal piles, grade i and grade ii can be managed conservatively with diet modifications and local applications of ointment. If still a problem persists there are several options like cryotherapy, sclerotherapy and laser treatment. But grade iii and iv require surgical intervention. In surgery open and closed hemorrhoidectomy, stapled hemorrhoidectomy are the options. AT AROGYAM CLINIC:- Grade I and II are managed conservatively with ayurvedic medication.
